Golden Thread (1)

The 3 pillars:

Develop a clear, consistent and motivating narrative (the golden thread) in order to effectively communicate the model of care and support its successful implementation

To co-design solutions to behavioural challenges identified by the system and place with a focus on embedding person centred care, prevention and self-care principals

To prioritise first steps and develop a programme of work for the implementation to lead to transformation not change for change sake
